Below are a range of screening tools for Avoidant/Restrictive Food Intake Disorder (ARFID). These have been developed by the Maudsley Centre for Child and Adolescent Eating Disorders, which are based at South London and Maudsley NHS Foundation Trust. Different versions are available depending on who is completing the screening: clinicians, parents/carers, or individuals themselves.

A single instruction guide is also provided to support the use of all screening tools.

About the Short ARFID Screen (SAS)

  • 7‑item, multi‑informant questionnaire
  • Based on DSM‑5 and ICD‑11 ARFID criteria; not a diagnostic instrument.

  • Suitable for children (2 +), adolescents and adults, regardless of body‑weight, to flag possible ARFID for further assessment or referral.

  • Includes basic demographics and questions that rule out other medical/mental causes, weight‑shape concerns, and assess functional impact.

  • Three formats:
